Authorities believe that the accident that claimed the life of Andy Kotowicz was caused by a driver who was speeding on 15th Avenue Northwest in Ballard, Washington, according to the Seattle Post-Intelligencer. The speeder reportedly crashed into one car, kept driving, then rear-ended Kotowicz's Subaru, causing a chain-reaction crash.

The news source reports that witnesses said that the driver who allegedly caused the crash appeared to be having a seizure after the incident.
Kotowicz was the head of marketing for Sub Pop records, reports the news provider.
"Andy was one of those people who oozed wisdom, warmth and kindness. He had a twinkle in his eye, a great sense of humor, and every time we talked, he had a fantastic new band to turn me on to," friend Kyla Fairchild wrote in a tribute, reports MyBallard.com.
