MCI/WorldCom.

A $750 Million dollar settlement has been reached between MCI (formerly WorldCom) and the Securities and Exchange Commission. The fine is 75 times larger than the previously largest fine of $10 million dollars levied by the SEC against Xerox Corporation for accounting irregularities.
